ALCHEMY
Referral Portal

Policies

Privacy

The Referral Portal coordinates referrals between healthcare professionals and community organizations without patient-identifying information. It is not designed to hold, and must not be used for, personal health information.

Coordination requests contain a short, non-identifying summary of needs and general details such as an age group and community. Names, health card numbers, dates of birth, addresses, diagnoses, clinical notes and filled-in forms must never be entered. Clinical referrals that need personal health information are completed outside the portal through approved secure channels.

The portal automatically screens requests and messages for common identifiers and asks the sender to confirm a request contains none. Anyone can report a privacy concern on a request; administrators review reports promptly, can remove identifying content, and keep a record of what was done.

We keep account details (name, work email and phone, role and organization), organization profiles and service listings, coordination requests and messages, and an audit log of actions taken in the portal. This information is used only to operate, secure and improve the portal and to produce aggregate (non-identifying) reports.

Notification emails contain only a reference and a link to sign in — never request content.

Terms of use

Accounts are for verified healthcare and community service professionals and organizations. Applications are reviewed before an account is created, and access may be suspended if these terms are not followed.

You are responsible for the accuracy of what you publish and for keeping patient-identifying information out of the portal. Keep your password private and sign out on shared devices.

Organizations are responsible for keeping their profiles, services, eligibility, catchment areas and blank forms up to date.

A coordination request — including an accepted one — is not a referral record, a clinical record, or proof that anyone received care.

The portal is not for emergencies. In an emergency, call 911. For mental health crisis support, call or text 9-8-8.

Accessibility

We aim for the portal to be usable by everyone, including people who use assistive technology: it supports keyboard navigation, screen readers, adjustable text size, and light and dark themes.

If any part of the portal is hard to use, please tell us and we’ll help — and use the feedback to improve it.
